Summary
This study tests a new computer system that helps pharmacists provide better care for people with COPD. The tool guides them through symptom checks, medication reviews, and creating action plans. Researchers will see if using the system in 6 pharmacies improves care compared to the year before. The goal is to learn if this approach can be used more widely to support pharmacists in managing chronic diseases.
