Summary
This study looks at whether a quick frailty screening tool (ISAR) can identify older adults in the emergency department who are at risk for medication problems, like harmful drug interactions or inappropriate prescriptions. Researchers will collect medical data from 300 patients aged 75 and older, without changing their care. If the frailty score works well, it could help pharmacists focus on the patients who need medication reviews the most.
