Summary
This study aims to create a digital system that identifies older adults at risk for disability and provides personalized interventions to prevent it. Researchers will enroll 238 community-dwelling adults aged 65 and older who can use a smartphone. The system will be tested in urban, county, and rural settings to see if it helps maintain physical function and independence.
