Summary

This study tested a new way of caring for young adults (ages 18-30) with type 1 or type 2 diabetes as they move from pediatric to adult diabetes clinics. 45 participants were split into two groups: one received the new collaborative care model, the other received usual care. Researchers measured how many visits patients completed and changes in diabetes-related distress and blood sugar control to see if the new model helps.
