New hospital program aims to keep COPD patients out of trouble after Flare-Ups

Summary
This study looks at whether a special care plan, called a transition bundle, can help people with COPD avoid serious health declines after a flare-up. About 1,368 patients from 36 hospitals will take part. Hospitals are randomly assigned to either use the new care plan or continue usual care. The goal is to see if the plan reduces worsening of COPD and helps patients stick to their long-term medications.
