Can family support keep seniors out of the hospital?

NCT ID NCT07661355

First seen Jun 23, 2026 · Last updated Jun 23, 2026

Summary

This study looks at two ways to help older adults after they leave the hospital. One program focuses only on the patient, while the other also supports family caregivers with education and skills. Researchers want to see which approach helps patients stay home longer and reduces hospital visits. Over 2,500 older adults and their families will take part.

What this could mean

Our plain-language read of the trial. This is informational only — not medical advice or a prediction.

Active substance

Patient-centered and family caregiver-enhanced transition programs

What this could lead to

If successful, this could show that adding family support to hospital discharge programs helps older adults stay home longer and reduces stress on caregivers.

What could go wrong

This is an early-stage study comparing two existing programs, so it may not find a clear winner. Results may not apply to all hospitals or patient groups.
