Summary
This study looks at two ways to help older adults after they leave the hospital. One program focuses only on the patient, while the other also supports family caregivers with education and skills. Researchers want to see which approach helps patients stay home longer and reduces hospital visits. Over 2,500 older adults and their families will take part.
