Summary

This study looked at whether community health workers (CHWs) can help low-income, mostly minority adults better manage chronic diseases like diabetes, high blood pressure, and obesity. Over 10 months, 202 participants either received CHW support plus standard care or just standard care. The goal was to see if CHW navigation improved health markers such as blood sugar, cholesterol, and stress levels.
