Philadelphia launches Community-Led fight against heart disease

Summary
This study is testing a new community health worker program to help people in Philadelphia better manage their risk for heart disease. The program combines group education sessions with one-on-one support to address both health behaviors and social needs like housing and food security. Researchers will enroll 500 local residents to see if this approach improves self-management skills more than standard services.
