New program aims to close gaps in kidney care for diabetics

Summary
This study is testing a new program designed to help doctors better screen for and manage chronic kidney disease in people with type 2 diabetes. It will compare a guided management program against basic education to see which leads to more people getting the right medications. About 420 adults with both conditions will be followed for about 6 months.
