New program aims to close gaps in kidney disease care for diabetics

Summary
This study is testing a new program designed to help doctors better screen for and manage chronic kidney disease in people with type 2 diabetes. It will compare a guided management program against standard education to see which leads to more people getting the right medications. About 420 adults with both diabetes and kidney disease will participate for about 6 months.
