Heart health coach for new moms: a simple way to lower blood pressure?

Summary
This study looks at whether adding a community health worker coach to remote blood pressure monitoring helps women who had high blood pressure during pregnancy better control their blood pressure and improve heart health after giving birth. About 240 women will be enrolled and followed for 6 months. The goal is to see if the coaching program leads to better blood pressure control, healthier habits, and fewer hospital visits.
