Summary

This study tested a dementia care program delivered by phone and web, where trained navigators helped patients and caregivers manage care. Over 1,200 people with dementia and their caregivers took part across six health systems. The goal was to see if this approach could improve quality of life, reduce emergency visits, and lower caregiver stress.
