Summary

This study is testing a program called CAPABLE, which is added to regular home-based primary care for older adults who are homebound and may feel socially isolated or lonely. The goal is to see if this combined approach can improve their daily function, reduce pain and depression, and increase social connections. About 450 participants from the Johns Hopkins Home-Based Medicine program will take part. This is a feasibility study, not a treatment trial, so it focuses on whether the program can be implemented and what effects it might have.
