Summary

This study tests whether a program called CAPABLE, when added to regular home-based primary care, can help older adults who feel socially isolated or lonely. Researchers will work with 450 homebound seniors to see if the program improves their daily function, reduces pain and depression, and boosts social connections. The goal is to find a practical way to improve quality of life for this vulnerable group.
