New program aims to ease cancer Patients' transition to nursing homes

Summary

This study tests a program called ALIGN, which provides virtual palliative care visits for older adults with advanced cancer who are moving from the hospital to a skilled nursing facility. The goal is to see if the program can be delivered successfully and help patients and their family caregivers feel more supported. Participants will have video visits with a social worker every 1-2 weeks and share how they are doing over 6 months.
