Summary

This study tests a program called ALIGN, which provides virtual palliative care visits for older adults with advanced cancer who are moving to a skilled nursing facility. The goal is to see if this approach is practical and helps patients and their family caregivers feel supported. Participants will have regular video visits with a social worker and share how they are doing over six months.
