Summary
This study aims to develop a computer prediction tool to identify which patients with Chronic Obstructive Pulmonary Disease (COPD) are most likely to be readmitted to the hospital soon after being discharged. Researchers will analyze the health records of up to 100,000 patients in Hong Kong to train a machine learning model. The goal is to create a system that can alert doctors to high-risk patients, potentially allowing for better follow-up care to prevent future hospital stays.
