Summary
This study aims to develop a computer prediction tool to identify which patients with chronic lung disease (COPD) are most likely to be readmitted to the hospital soon after being discharged. Researchers will analyze the health records of 100,000 patients in Hong Kong using machine learning to spot patterns that lead to early or frequent readmissions. The goal is to create a tool that helps doctors provide better follow-up care to the patients who need it most.
