Summary
This study tests whether a smartphone app powered by artificial intelligence can help adults aged 18-45 with early heart disease avoid serious problems like heart attacks or strokes. About 4,900 participants will use the app to track their health, get reminders, and receive personalized advice on diet, exercise, and medications. The goal is to see if the app leads to fewer hospital visits and better control of risk factors compared to standard care.
