Summary
This study tests whether a smartphone app powered by artificial intelligence can help adults aged 18 to 45 with early heart disease manage their condition better than standard care. Participants use the app to track lifestyle habits, receive reminders, and share health data with their care team. The goal is to see if the app reduces the chance of death, heart attack, stroke, or unplanned hospital visits within one year.
