This study tested whether a web-based educational tool called 'Prepare for Your Diabetes' could help older adults (75+) with type 2 diabetes talk to their doctors about adjusting their medications to prevent dangerously low blood sugar. The study involved 669 participants from Kaiser Permanente Northern California who were at high risk for hypoglycemia. The goal was to see if this approach reduces hospital visits and severe low blood sugar episodes.
