This study tested a web-based educational tool called 'Prepare for Your Diabetes' in 669 older adults (75+) with type 2 diabetes who were at risk for low blood sugar. The tool helped patients discuss their treatment goals with their doctor to reduce risky medications. The main goal was to see if this approach lowered the number of severe low blood sugar events over 12 months.
