Summary
This study aims to find the best ways to help teens and young adults with type 2 diabetes get support for basic needs like food, housing, and transportation. Researchers will test different approaches, including text message reminders and connections to community health workers, to see which methods are most practical and acceptable to families. The goal is to learn how to better connect young patients with resources that can help them manage their health.
