Summary
This study tests the WHO's ICOPE program, which screens older adults for early signs of decline in mobility, memory, nutrition, vision, hearing, and mood. If problems are found, a personalized care plan is created. The goal is to see if this approach can prevent or delay loss of independence compared to usual care. About 1,000 people aged 65 and older who already show mild decline in at least two areas will take part for up to 5 years.
