Summary
This study tests a personalized care plan for people leaving the hospital after acute kidney injury (AKI). About 6,000 adults in Alberta hospitals will be randomly assigned to either receive a tailored care plan based on their risk or usual care. The plan uses digital tools in the electronic health record to guide follow-up and aims to reduce serious problems like kidney failure, heart attacks, and death.
