Summary

This study tests a 6-month program called Type 1 Together for teens aged 12-17 with type 1 diabetes who don't use a continuous glucose monitor (CGM) regularly. Participants get monthly meetings with a diabetes expert, access to educational app resources, and a mentor family to help them use CGM more consistently. The goal is to see if the program improves CGM use, attitudes, and blood sugar control.
