Summary
This study looks at a special clinic designed to help adults with conditions like cerebral palsy, spina bifida, or developmental disabilities get better, ongoing care. Many of these people face a "cliff" when they leave children's healthcare and struggle to find adult doctors who understand their needs. The clinic brings together a team of experts to manage their physical, mental, and social health. Researchers will study if this model improves patient satisfaction and quality of life for about 410 participants in Ontario.
