Summary

This study is testing whether sending personalized text messages can help older adults with heart failure take better care of themselves. Researchers will enroll 30 participants who will either receive about 5 supportive texts per week for 12 weeks or be in a comparison group. The goal is to see if these reminders about medications, diet, and symptom monitoring make self-care easier and more effective.
