App aims to keep kidney patients out of the hospital

Summary

This study is testing whether a home monitoring system can help people with advanced kidney disease avoid emergency room visits and hospital stays. About 340 patients will use a tablet app to track their blood pressure, weight, and symptoms daily, sharing the data with their care team. The goal is to see if this extra monitoring helps catch problems early and improves patients' sense of safety and quality of life.
