Summary

This study is testing whether remotely mentoring local clinic teams can help them successfully start a comprehensive diabetes care program. The program includes telehealth support, community health workers, and education specifically for low-income, Spanish-speaking Latino adults with type 2 diabetes. Researchers will compare health outcomes like blood sugar control and blood pressure between people in the new program and those receiving usual clinic care over three years.
