Summary
This study tests whether proactive telemedicine (brief phone or video calls) can help rural patients who haven't seen a doctor in over a year get back into preventive care. About 120 adults will be randomly assigned to receive either remote coaching on healthy habits (like quitting smoking or eating better) or standard in-person visits. The goal is to see if telemedicine is as effective as face-to-face care for improving health access and quality of life.
