Summary
This study is testing whether using telemedicine (remote monitoring via phone or tablet) can help doctors safely adjust heart failure medications for patients after they leave the hospital. It compares this remote approach to usual care to see if it leads to better medication use, fewer hospital readmissions, and improved quality of life. The goal is to provide closer support during the vulnerable weeks after discharge without requiring as many in-person visits.
