Summary

This study tests whether a telehealth recovery clinic can help adults aged 45 and older who survived a stay in the intensive care unit (ICU) recover their thinking skills and ability to perform daily tasks. Participants will have a video visit with a team that includes a doctor, a mental health expert, and a case manager. The goal is to see if this approach improves cognitive function and independence compared to usual care.
