Summary

This pilot study tests whether a telehealth follow-up program for people who survived a serious illness in the ICU is practical. About 24 adults who needed a breathing machine or stayed in the ICU for at least a week will be randomly assigned to get standard care or extra video visits with a pharmacist at 1 and 3 months after leaving the hospital. The goal is to see if this approach can help spot and manage ongoing health problems after the ICU.
