Summary

This pilot study tests whether a pharmacist-led telehealth follow-up program is feasible for people who survived a critical illness in the ICU. About 24 adults who needed a breathing machine or stayed in the ICU for at least a week will be randomly assigned to receive either standard care or telehealth visits at one and three months after leaving the hospital. The goal is to see if this approach can be done smoothly, not yet to prove it improves health.
