Summary

This pilot study is testing whether a 12-week telehealth support program can help frail older adults recover better after a hospital stay. The program provides weekly video sessions and uses activity sensors to encourage movement and monitor progress. Researchers will enroll 50 participants to see if the program is practical and if it improves physical fitness, mood, and reduces return trips to the hospital.
