Summary
This pilot study tests a 12-week telehealth program for frail adults aged 65 and older who have just left the hospital. The program uses activity sensors and pedometers to support physical and mental recovery at home. Researchers will check if the program is practical and liked by participants, and measure its effects on fitness, daily activities, mood, and hospital visits. The goal is to gather information for a larger future study.
