Summary
This study is testing whether providing a continuous glucose monitor and a dedicated Patient Navigator can help people with type 1 diabetes who have struggled with severe complications or high blood sugar. It aims to see if this combination of technology and personal support improves how people manage their diabetes and their quality of life. The goal is to address healthcare disparities and promote more equitable outcomes.
