New team approach aims to tame diabetes in vulnerable communities

Summary

This study is testing whether a support team can help people with type 2 diabetes use continuous glucose monitors (CGMs) more effectively to control their blood sugar. It will involve 318 diverse, low-income patients who are not using insulin. The research will see if adding a clinical pharmacist and, if needed, a community health worker to guide CGM use leads to better health outcomes and quality of life over 12 months.
