Summary

This study is testing whether a simpler, easier-to-read survivorship care plan helps Black and African American men who have finished treatment for prostate cancer. Researchers want to see if this tailored plan helps patients understand their follow-up care and manage long-term side effects better than a standard plan. The goal is to improve health outcomes by making important medical information more accessible.
