First seen May 05, 2026 ยท Last updated May 05, 2026
Summary
This study tests whether a transition coordinator can help 324 youth with type 1 diabetes (ages 17-18) successfully move from pediatric to adult diabetes care. Participants get extra support before, during, and after the switch. Researchers compare them to a group that didn't have this help, looking at clinic attendance, diabetes control, and quality of life.
