Summary
This study tested whether adding a special care model (GRACE) to regular annual wellness visits helps older adults with complex health needs. Researchers worked with 110 people aged 65 and older to see if this approach is practical and could reduce hospital stays. The goal was to improve patient and caregiver experiences, not to cure a disease.
