New program aims to bridge the gap from hospital to home for substance use recovery

Summary
This study tests whether a special team of doctors, nurses, and peer supporters can help people with substance use disorders get the follow-up care they need after a hospital stay. The team meets patients in the hospital and then provides support and treatment after discharge while connecting them to long-term community care. The study will include 75 adults and measure how many attend follow-up visits and stay in treatment.
