Summary

This study aims to find out if a quick frailty screening tool used in the emergency room can identify which elderly patients are most likely to have unsafe or inappropriate prescriptions and dangerous drug interactions. Researchers will collect medical data from 300 patients aged 75 and older to see if a higher frailty score predicts more medication-related problems. The goal is to give pharmacists a simple way to prioritize which patients need an urgent medication review to improve safety.
