Summary
This study tests a 5-week support program for stroke survivors after they leave the hospital. The goal is to see if the program helps them feel more confident and independent in managing their daily health. Researchers will compare participants who get the program plus usual care to those who only get usual care. The study involves 140 adults who have had a stroke and are stable enough to participate.
