Researchers and stroke survivors team up to fix the 'Missing Link' in recovery
NCT ID NCT02925871
Summary
This project aimed to create and test a better system for helping stroke patients move from the hospital back to their homes. Researchers worked directly with stroke survivors, their families, and healthcare staff to design a more personalized and coordinated transition plan. They first studied the current process to find problems, then built a new approach, and finally tested it with a small group to see if it was practical and helpful.
