Summary
This study tests whether a WeChat-based intelligent management system can help people who have had a stroke or mini-stroke avoid another one. About 4,490 participants will either get usual care or use the app after leaving the hospital. After one year, researchers will check for new strokes, medication use, and how well risk factors like blood pressure are controlled.
