Summary

This study tests a program called SPARK-CGM that helps primary care doctors learn about and prescribe continuous glucose monitors (CGMs) for people with diabetes. CGMs track blood sugar in real-time, helping patients avoid dangerous lows and improve their quality of life. The study will involve 20,000 patients across many clinics to see if the program increases CGM use and makes it more accessible for everyone.
