Summary

This study is testing if 'social prescribing' can help women with heart failure take their medications and feel better. Social prescribing connects patients with a 'link worker' who helps them find community services for needs like transportation, housing, or loneliness. Researchers want to see if this personalized support improves medication adherence and quality of life for 200 women across Canada.
