Yale tests new approach: community helpers for diabetes care

Summary
This study is testing whether connecting young adults with type 1 diabetes to a community health navigator helps them resolve social needs like housing or food insecurity. Researchers will compare 100 participants who receive this support to those who continue with usual care. The goal is to see if this approach reduces diabetes distress and improves blood sugar control over six months.
