Summary
This study tests a digital tool called "Smart family doctor" that uses artificial intelligence to help patients manage their blood pressure, blood sugar, and cholesterol after heart surgery or stent placement. About 951 adults who have had bypass surgery or a stent and also have high blood pressure, diabetes, and high cholesterol will use the tool to see if it helps them reach their health goals better than standard care.
